As a Referrals Nurse at Bleep 360 Care, you will play a crucial role in assessing, prioritising, and coordinating care for individuals requiring our services. This role is ideal for a registered nurse with strong clinical judgment, excellent communication skills, and the ability to work under pressure while maintaining high standards of care and patient safety. A thorough understanding of CQC regulations, safeguarding, and best practice clinical governance is essential.
You will be responsible for conducting initial clinical assessments, identifying urgent and routine care needs, and ensuring appropriate pathways are followed for safe and effective care delivery. Your role will involve working closely with multidisciplinary teams, supporting service users and their families, and ensuring all triage decisions are accurately documented and aligned with regulatory and clinical governance requirements. You will also assist with audits, contribute to ongoing quality improvement initiatives, and support compliance processes to uphold the highest standards of care.

Duties/Responsibilities:
Audit & Quality Assurance
Assist in conducting clinical audits, gathering and analysing data to assess the effectiveness of triage decisions and patient outcomes.
Ensure that all triage-related documentation, including assessment records, risk assessments, and care pathways, is maintained accurately and up to date.
Support continuous service improvement through data collection, reporting on key quality metrics, and identifying trends in service delivery.
Triage & Information Management
Maintain accurate and timely clinical documentation, ensuring clear, comprehensive, and legally compliant patient records.
Share essential updates and clinical bulletins with staff via the staff intranet, ensuring timely access to triage protocols and best practice guidelines.
Contribute to the monthly publication of clinical updates, ensuring all triage nurses are informed of regulatory changes and service improvements.
Clinical & Team Support
Provide specialist triage support to frontline teams, ensuring efficient coordination of urgent and routine referrals.
Assist in the scheduling and minuting of clinical governance and quality meetings, ensuring actions are documented and followed up appropriately.
Support the rollout of new policies and procedures, ensuring triage teams understand and implement best practice standards effectively.
Skills/Experience Required:
Clinical & Healthcare Experience: A strong background in nursing (ideally RMN), ideally with experience in triage, community mental health nursing or similar clinical settings. NMC Pin required.
Regulatory Knowledge: Experience working within a CQC-regulated environment, with a solid understanding of clinical governance, safeguarding, and regulatory frameworks in health and social care (desirable).
Assessment & Decision-Making: Proven experience in clinical assessment, prioritisation, and risk management, ensuring safe and effective triage decisions.
Audit & Compliance: Previous experience supporting clinical audits, ensuring adherence to policies and procedures, and contributing to service quality improvements.
Data Management & Documentation: Experience handling confidential patient information, maintaining accurate triage records, and working with electronic patient record systems to support quality assurance.
Communication & Coordination: Strong written and verbal communication skills, with experience in liaising with multidisciplinary teams, documenting assessments, and participating in clinical meetings.
IT Proficiency: Competent in using Microsoft Office and clinical software (training provided)
Attention to Detail: A track record of working with accuracy and efficiency in a fast-paced clinical environment, ensuring triage documentation and patient records are well-organised and up to date.
