The Opportunity:

This is a rewarding and varied role where no two days are the same. You’ll play a vital part in ensuring our services are safely staffed, well-organised and delivered to the highest standards.

You’ll work closely with our Registered Manager, Clinical Team and external professionals to coordinate person-centred care that truly makes an impact.

What You'll Be Doing:

  • Coordinating care packages and staffing to meet individual needs

  • Creating and reviewing person-centred care plans

  • Acting as a key point of contact for families and professionals

  • Attending safeguarding and care review meetings

  • Ensuring services remain compliant with CQC standards

  • Supporting care staff with guidance and oversight

  • Contributing to audits and continuous service improvement

  • Support our network of property landlords across the UK, identify suitable accommodation and suggest adaptations to accommodate service users living there

What We’re Looking For:

  • Previous experience (minimum one year) in care coordination, housing management, welfare/benefits support, or a related health and social care profession would be advantageous.

  • Commercial background / sales background
  • Strong organisation and communication skills

  • A proactive, solution-focused mindset

  • Ability to remain professional in challenging situations

  • Understanding of CQC standards

  • This role involves direct work with vulnerable individuals. A satisfactory Enhanced DBS check is required prior to employment
